After the appointment is complete and if you are approved, the doctor will fill out a recommendation form for medical marijuana.  As soon as your practitioner has certified that you have a qualifying condition, you will receive an email from us to the email address you provided to us.  You may need to check your spam/junk folder.  Once you have this you can then apply to the state.

Individuals applying for a medical card must have a physician’s recommendation signed by a Montana licensed physician (Medical Doctor or Doctor of Osteopathy) within 60 days of application submission.
