In Utah, the Medical Cannabis Act specifies certain medical conditions that qualify a patient for a medical marijuana card. The qualifying conditions include:
Alzheimer’s Disease

Autism

Cancer
Chronic Pain: Defined as pain lasting longer than two weeks that is not adequately managed despite treatment attempts using conventional medications other than opioids or opiates or physical interventions. Utah Medical Cannabis Program
Crohn’s Disease
Epilepsy or Debilitating Seizures
HIV/AIDS
Multiple Sclerosis (MS) or Persistent and Debilitating Muscle Spasms
Persistent Nausea: Excludes nausea related to pregnancy, cannabis-induced cyclical vomiting syndrome, or cannabinoid hyperemesis syndrome.
Terminal Illness: With a life expectancy of less than six months.
Conditions Requiring Hospice Care
Rare Conditions or Diseases: Affecting fewer than 200,000 individuals in the U.S., as defined by federal law.
Having a “qualifying medical condition” in Utah is the first step, but it doesn’t automatically make you a qualified patient. Here’s what you need to do next:
Visit a Qualified Medical Provider (QMP): You’ll need to consult with a provider who is registered with Utah’s medical cannabis program. During this appointment, they’ll assess whether medical marijuana is appropriate for your condition.
Get a Recommendation: If your QMP determines that medical cannabis may benefit you, they’ll submit a recommendation through Utah’s electronic verification system.
Apply for a Medical Cannabis Card: After your provider’s recommendation, you’ll need to complete an online application through Utah’s Department of Health. This includes paying a fee and submitting any required documentation.
Approval and Card Issuance: Once your application is reviewed and approved, you’ll receive your medical cannabis card, which allows you to purchase cannabis from licensed dispensaries.
So, even if you have a qualifying condition, the additional steps with a QMP and application are necessary to become a fully qualified patient.
Step 1. Book. Book an Appointment Online
Step 2. Payment. Pay Online
Step 3. Sign an Online Consent Form (a link will be emailed to you upon payment)
Step 4. Submit your Qualifying Medical Records

Obtain Certification from Your QMP: After your consultation, your QMP will enter their recommendation into the EVS, certifying you for medical cannabis use.
Pay the Application Fee: Once certified, log back into the EVS to pay the $15 application fee. This fee is required to process your medical cannabis card.
